CHECKLIST FOR NEW STUDENT-ATHLETES

 

First-Time Freshmen Only

The NCAA requires that all high school students must apply to the NCAA Clearinghouse for athletic eligibility. Call (319) 337-1492 with questions or access the website: http://eligibilitycenter.org. Students should remind their high school counselors to send the yellow Clearinghouse form to the Clearinghouse along with their final transcripts and test scores.

Working high school transcripts sent to Matt Steuart or Quinton Freeman in the Student Support Office, 921 S. 8th Ave. Stop 8173, Pocatello, ID 83209.  Phone:  (208)-282-4668.

Seventh semester high school transcript sent to ISU Admissions (address above).  Attention Gene Denning.  Phone:  (208)-282-3304.

  Eighth semester final official high school transcript with graduation date posted should be sent directly from your high school registrar’s/records office to ISU Admissions Office immediately following graduation.

On a National Testing day, ACT/SAT taken and forwarded by ACT/SAT testing agencies to ISU Admissions Office (address above).  ACT phone:  (319)-337-1313, ISU code:  0918.  SAT phone:  (609)-771-7600, ISU code:  4355.

Housing application and $150 deposit mailed to ISU Housing, 921 S. 8th Ave., Stop 8083, Pocatello, ID 83209.  Phone:  (208)-282-2120.

  Free Application for Federal Student Aid (FASFA) completed and mailed in envelope enclosed in Aid Packet OR completed and submitted online at www.fafsa.ed.gov.  Request that a copy of the results be sent to Idaho State University, #001620, Pocatello, ID 83209.  Financial Aid phone:  (208)-282-2754.

  Send the following documents to the NCAA Eligibility Center:  1) Completed and signed Student Release Form and fee (www.ncaaclearinghouse.net), 2) Official, final transcript sent directly from high school, 3) ACT or SAT scores sent from testing agencies, Clearinghouse code: 9999

Junior College/4 Year Transfer Student

  Application for Admission completed and forwarded to ISU Admissions Office, Campus Box 8270, Pocatello, ID, 83209.    Attention: Carole Hagler.  Phone: (208) 282-3296, Fax: (208) 282-4511.  Online: www.isu.edu.

$40 application fee forwarded to ISU Admissions Office (see above)

  Working college transcripts sent to Matt Steuart in the Student Support Office, Campus Box 8173, Pocatello, ID, 83209.  Phone:  (208) 282-2575

  Final official college transcripts with degree posted, if appropriate, should be sent directly from the College Registrar/Records Office to the ISU Admissions Office (see above).

  Housing application and $100 deposit mailed to ISU Housing, Campus Box 8083, Pocatello, ID, 83209.  Phone:  (208) 282-2120.  Off-campus arrangements are the responsibility of the student.

  Free application for Federal Student Aid (FAFSA) completed and mailed in envelope enclosed in aid packet.

  Financial Aid transcripts sent from your college to ISU Financial Aid Office, Campus Box 8077, Pocatello, ID, 83209.  Phone:  (208) 282-2756.

If you did not receive an Associate’s Degree (AA), the following actions must be taken.

   ACT/SAT taken before high school graduation and official score results sent from ACT/SAT to Admissions Office (see above).  ACT Phone: (319) 337-1313, ISU Code: 0918.  SAT Phone: (609) 771-7600, ISU Code: 4355.

   Final official high school transcript with graduation date posted, sent to ISU Admissions Office (see above).

Send the following documents to the NCAA Clearinghouse:

  Completed and signed Student Release Form and fee (www.ncaaclearinghouse.net).

  Official, final high school transcript, showing graduation date

  ACT or SAT scores, Clearinghouse Code: 9999

  48-H Form

Note:  In order to be considered for New and Transfer Student Academic Scholarships at Idaho State University, your Application for Admission, $40 Application Fee and official high school transcript must be sent to the ISU Admissions Office, postmarked by February 1, 2011.
